Attached here are the DOE guidelines for this upcoming school year. Please review so you are aware of the expectations and what may occur this school year as we return to school in this pandemic.
Afternoon. Chancellor Porter just announced that all students participating in a high-risk sport or in a facility that requires vaccination, will need to be vaccinated to participate. You can read the memo here. In order to get your child (12 or older) vaccinated, go to vaccinefinder.nyc.gov to schedule an appointment. You can also call 877-VAX-4-NYC (877-829-4692) for help making an appointment at a City-run vaccination site.
Fall sports season is here. Practices for football, girls’ volleyball, cross country track & boys’ soccer will begin Sunday, August 15th or shortly after. For students to participate in any sport activities, there are 3 forms that must be completed and submitted to coaches. Those are:
1. Parent Consent Form
2. Pre-Participation Exam Form
3. Consent to COVID-19 Testing
Use the links to get a digital copy of each form.
